Account Setup
To ensure a HIPAA-compliant experience, please register for your patient portal account. Through the portal, you can securely access prescriptions, invoices, and records from our office, and communicate with us via web messages from any device.
An email registration link will be sent to you where you can view your prescriptions, patient care and office forms, invoices and most importantly, it will also allow us to communicate through the portal.
​​1. Access the patient portal
2. Click "register new account"
3. Enter the email on file
4. Check your inbox from a do_not_respond crystal pm domain and click "set password"
5. Your default user name is Lastname_Firstname (full name). Log in with your username and new password (case sensitive)

For convenience, please fill out the necessary forms for your appointment. New patients are required in order to be scheduled and confirmed.
Having Trouble Logging In?
If you're having difficulty signing in with your username or password, it’s possible that your registration link was not sent or completed.
If you’ve accessed your account previously, you can reset your password or request a password reset link. Please ensure you enter your login details exactly as they were registered — usernames and passwords are case sensitive. If you're still unable to access your account, please contact us.
Having trouble submitting forms?
If you're having difficulty submitting forms in the portal, please make sure to click the "Submit Data" tab and then the submit button to ensure your form is successfully sent.
